This is not an official test or diagnosis for children. It is a parent-reported pre-screen based on recent observations: ADHD-like difficulties, autism / sensory load, anxiety, OCD repetitions, low mood, and emotion regulation. If overlapping patterns appear, differential questions open.
Why a separate form?
Adult items use work, masking, and hypomania language. The child form uses school, play, routine, and caregiver observation. Bipolar screening is cautious; it rarely presents prominently in young children.
